To ensure your student is eligible for athletic participation, the following steps must be completed:

  1. Physical Form:
    Upload an up-to-date physical (both front and back pages) to the correct Physical section in Aktivate. Incomplete or incorrectly placed documents will delay clearance to play.
  2. Insurance Information:
    • If you are not purchasing HISD insurance:
      • Upload both the front and back of your insurance card, with the student-athlete’s name clearly visible to the Insurance Card section.
      • Upload a notarized insurance waiver to the designated area if you are choosing to use your own insurance.
    • If you are purchasing HISD insurance:
      • Submit the $35 payment via SchoolPay.
      • Upload the receipt from SchoolPay in the Insurance Card section of Aktivate.
  3. UIL Documents:
  • Complete all required UIL forms/registrations for your student’s sport directly in Aktivate.

Please take a moment to verify that all documents are current, complete, and uploaded to the appropriate sections. Incomplete profiles may result in delayed participation, including missed tryouts, and loss of athletic participation for the 2025-2026 school year.
